MyCareTeam is comprehensive in supporting participants living with disability set goals, achieve growth and becoming economically active members of society. The participants are supported by a number of health care professionals, family and friends. All goals created are monitored using qualitative and quantitative data inputs, categorized into high, medium and low priority. The MyCareTeam App also enables all registered users stay connected through the in-app messaging tool.
